Who receives a contact inquiry?
Credentialing Reviews is operated by Alevated Holdings LLC. Contact inquiries are collected through Welaka and sent to the Alevated team. The form requests your name and email, with optional phone and message fields. The team uses this information to handle your request, including corrections and updates to company listings.
If you request contact with a listed company, Alevated may relay your contact details and inquiry to that company so it can respond. Name the company in your message and include only information you want to share for that request. The receiving company handles the forwarded information under its own privacy practices.
Contact messages are not automatically published as reviews or listing changes. Please do not send patient information, passwords, identity documents, or confidential contracts.
What happens when you browse?
Cloudflare hosts this site and processes the technical requests needed to deliver it. On credentialingreviews.com, Google Analytics measures site visits and successful contact submissions. The site's lead event does not include the name, email, phone number, or message you enter.
The contact form loads code from Welaka and sends the page address, referring page, and campaign parameters when available alongside your inquiry. After a successful submission, this site stores a temporary marker in your browser's session storage to avoid counting a direct thank-you-page visit as a lead. The marker is accepted for ten minutes and removed when the confirmation page checks it. Your shortlist selections are stored in the comparison page address as company identifiers, so anyone you share that link with can view the selected companies.
Can I submit a review or company draft?
The form checks whether collection is open before offering submission. During setup, you can prepare and download a local draft without sending it. A downloaded draft remains on your device; it is not a submission receipt.
While direct form submission is unavailable, you can copy a completed review or company request into an email to [email protected] for editorial consideration. Your email provider sends that message; this site cannot confirm its delivery. Alevated Holdings LLC handles directory inquiries. Contact or email messages are not automatically published, verified, or forwarded to a listed company.
What information does the form collect?
Review forms request a public display name, private email address, company, role, engagement month, rating, service used, experience, and relationship disclosures. Company forms request a representative’s name and email, company website, scope, coverage, and supporting public sources.
Do not include patient information, passwords, confidential contracts, or identity documents. There is no document-upload field. The draft download includes the information you entered, so store it appropriately.
What may become public?
Approved review content may include the display name, role, service, engagement month, rating, experience, and disclosed relationships. Email addresses, abuse-prevention records, and private moderation notes are excluded from public exports. Company descriptions and responses are attributed separately.
The server keeps new submissions in a moderation queue; it does not automatically publish them. When enabled, spam protection checks requests through Cloudflare and uses a hashed network identifier for request limits. These review-intake controls are separate from the active Welaka contact form.
How do I ask about my submission?
Email [email protected] to ask about information you submitted, request a correction or deletion, or ask about retention. Include enough context to identify your inquiry without sending additional sensitive information. A fixed retention period for contact inquiries has not been published. If your request was forwarded to a company, contact that company about its copy as well.
